New Patient Forms

We are excited that you would consider honoring us with being your partners in health!   At our Essential Psychiatry department, we aim to help you manage and get through your mental anguish and provide you with the skills and support to help you find your success. We are solution focused, LGBTQ friendly and here to serve you.

In order to reduce your wait time to see one of the providers, we encourage you to fill out our forms.  As you know, paperwork is always the longest part of any visit to any new healthcare facility, and we would like to make your visit as smooth as possible, allowing us to focus on YOU, not your paperwork.

When you schedule your first appointment, we will email and/or text you a link where you can fill in these forms online. Simply go through and when you’re done, submit them. There’s no need to print them out, no need to bring them in. This saves us both time, and enables us to get you in for treatment when you’re in.
